Patient Referral
How Can I Get an Appointment?
All patients need to be referred by a medical doctor in Ontario. To make the most of our first appointment please have your doctor send the following:
1) Full demographics, so we know how to best contact you
2) Your updated health card information
3) Previous notes including:
- All neurologists and specialists notes
- Any other relevant medical history
- MRI reports
- EEGs

Also, to make most of your first appointment you may be asked to fill out a questionnaire before coming to the clinic and to hand it into the receptionist upon your arrival.
Please bring all your current medications including prescription and non-prescription (vitamins, herbal products etc).
You may be asked to bring an MRI on a CD.
You may also be asked to bring a witness to the episodes who can describe what they have seen and/or provide a video of the episode. This will help improve the diagnosis.

*Please note our Conduct and Communication Policy: Since, COVID-19 is winding down, we understand patients can get frustrated with wait times. Due to healthcare constraints there are many demands on our staffs’ time and resources. Therefore, we ask that you be respectful to all our staff in all situations. Be it in person, over the phone or any means of communication.
Our clinic is a professional environment and we are here to assist all our patients. We thank you for your cooperation and look forward to continuing a respectful relationship with all our patients.
Cancellation Policy
*Please note our Cancellation Policy: To provide timely care we ask our patients to provide us with at least 48 hours notice. There are fees for those who do not show up for their scheduled appointments or those who do not cancel within an appropriate time.
Is this covered?
All doctor visits are covered by your provincial insurance plan (ex. OHIP in Ontario)
Patients who do not have any provincial coverage can be seen as well, although there is a fee. If you have private insurance (ex. student insurance) this can be reimbursed. Please contact us for details.

Please note not all services are covered. There are fees for non-OHIP services which includes insurance forms, legal letters. sick notes, etc. Please contact our office for more details.
